Student Information Card: (this is a card you will use to gather information for use on the docs.bigcreekmissions.com website. The ministry track signup is only a preliminary signup. The official signup for ministry tracks occurs when your group arrives in Kentucky.)

Adult Information Card: Please have all adults attending the mission fill out one of these forms and mail / fax / email these 25 days prior to your trip to Big Creek. These cards will allows us to inform your adults of their ministry track assignments prior to arrival. We will also inform construction crew leaders of their basic assignment.

Staff Applications: For those interested in serving as staff or SLA's at Big Creek.
Note: Medical Needs: Because we may or may not have a nurse on site during the week, it is the group leaders' responsibility to provide for the special needs of students (examples: allergies, medication, special illnesses). FYI: Three hospitals with emergency services are within short driving distance: Manchester Memorial (15 minutes), Appalachian Memorial / Hazard, (25 minutes) and Mary Breckenridge in Hyden (20 minutes).

Big Creek Missions received a wonderful grant from the Tennessee Baptist Foundation that will enable us to purchase some much needed tools, trailer, and truck for construction ministries!
